Metabolic Panel

The Inborn Errors of Metabolism (IEM) Genetics Panel is a targeted Next-Generation Sequencing (NGS) assay engineered to comprehensively profile germline mutations across genes encoding metabolic enzymes, transport proteins, and cofactors involved in cellular bioenergetics, carbohydrate, amino acid, lipid, and nucleic acid pathways. Metabolic disorders arise from functional enzymatic deficiencies leading to toxic substrate accumulation or critical energy failure. This panel resolves diagnostic ambiguity in patients presenting with unexplained neurodevelopmental regression, intractable seizures, metabolic acidosis, recurrent hypoglycemia, hyperammonemia, or organomegaly, enabling prompt molecular diagnosis, targeted dietary/enzymatic management, prevention of irreversible organ damage, and cascade family testing.

Key Features & Advantages

  1. Rapid & Life-Saving Diagnostics: Accelerates molecular diagnosis during acute neonatal or pediatric metabolic crises to prevent permanent neurodevelopmental impairment and organ failure.
  2. Directs Precision Nutrition & Enzyme Replacement: Pinpoints exact enzymatic defects to immediately guide substrate-restricted diets (e.g., PKU, MSUD), cofactor supplementation, or Enzyme Replacement Therapy (ERT).
  3. Overcomes Biochemical Diagnostic Limitations: Provides definitive genetic resolution when biochemical assays (e.g., tandem mass spectrometry, organic acids) yield ambiguous, borderline, or state-dependent results.
  4. Enables Family Planning & PGT-M: Given that most IEMs follow autosomal recessive or X-linked inheritance, identifying pathogenic variants allows carrier screening and informs preimplantation genetic testing (PGT-M) or prenatal diagnosis.

Metabolism NGS Panel vs. Conventional Biochemical Screening

FeatureMetabolism Genetics Panel (NGS)Standard Biochemical Screening (Tandem MS, Organic Acids)
Diagnostic ScopeGenes encoding metabolic enzymes, transporters, and cofactorsConcentrations of circulating or excreted metabolites
Etiological ResolutionHigh (Identifies the exact gene mutation and enzymatic flaw)Phenotypic (Detects metabolite alteration, subject to secondary elevations)
Sample Timing SensitivityIndependent (Unaffected by fasting state, diet, or clinical crisis)Highly Dependent (Often requires sampling during acute crisis/fasting)
Pre-Symptomatic DetectionYes (Identifies genetic risk prior to metabolic decompensation)Limited (Metabolite accumulation occurs post-pathological blockade)
Reproductive GuidanceHigh (Provides specific molecular target variants for PGT-M)Low (Cannot be used directly for PGT without gene identification)

Key Applications

  1. Aminoacidopathies & Urea Cycle Disorders: Phenylketonuria (PKU), Maple Syrup Urine Disease (MSUD), Tyrosinemia, and Urea Cycle Defects (OTC, ASS1).
  2. Organic Acidemias & Fatty Acid Oxidation Defects: Methylmalonic Acidemia, Propionic Acidemia, Isovaleric Acidemia, and MCAD/VLCAD deficiencies.
  3. Lysosomal & Peroxisomal Storage Diseases: Gaucher Disease, Fabry Disease, Pompe Disease, Mucopolysaccharidoses (MPS), and Zellweger Spectrum Disorders.
  4. Glycogen Storage Diseases & Carbohydrate Disorders: Glycogen Storage Diseases (GSD subtypes), Galactosemia, and Hereditary Fructose Intolerance.
  5. Mitochondrial & Energy Metabolism Disorders: Pyruvate Dehydrogenase deficiency, Succinate Dehydrogenase defects, and respiratory chain complex deficiencies.

The Nucleus Genetics Advantage: Combining broad target NGS panels customized for metabolic genetics, advanced bioinformatic pipelines aligned with ACMG and SSIEM guidelines, and expert curation by medical geneticists and biochemical specialists, we transform complex metabolic genomic data into actionable clinical insights.
